WHO WE ARE

The Community and Homeless Health team has a wealth of experience supporting individuals at-risk of or experiencing homelessness in a variety of capacities and settings including but not limited to: street medicine and mobile outreach, outpatient care coordination, shelter coordination, quantitative and qualitative research, and healthcare and housing program administration.

Our collective experience and strategy for partnership and action is grounded in service, empathy, equity, compassion, humility and accountability.

Ashley Brand, MA, MPH, CPH

System Director, Community Health, Integration & Housing

Ashley's portfolio focuses on addressing housing insecurities, homelessness and food insecurities across our national footprint. Her priorities include the development and implementation of the California homeless health initiative which concentrates on transforming health and our communities by strengthening the continuum of care for those experiencing housing insecurities and homelessness through innovation, partnership and lasting impact and the development of a national food system strategy. Ashley has over 15 years of experience developing and implementing programs that target the underserved nationally and internationally.

Nicole Wilson, MSW

Community and Homeless Health Project Manager (Southern California)

Nicole’s portfolio focuses on addressing housing insecurities and homelessness prevention across CA which ranges from managing a program focused on serving patients who are chronically homeless and frequent users of hospital services to supporting the creation of an advisory council of individuals with lived expertise of homelessness. Her priorities include elevating awareness for needed services for those who are historically under-represented. Nicole’s professional background includes coordinating LA County’s Demographic & Youth Count and serving as a research consultant for projects focused on youth experiencing homelessness.

Anira Khlok

Community and Homeless Health Project Manager (Northern California)

Anira’s portfolio focuses on homelessness prevention and response projects across CA from permanent housing, medical legal partnership, medical respite to mobile outreach. Her priorities include elevating effective and data-informed community-based interventions that can be scaled and replicated to address structural issues exacerbating homelessness and health inequities. Anira's professional background includes multi-sectoral service coordination, global health, outpatient social work and project management.
